In case of an emergency, (enter name of temporary custodian) has my consent to approve medical care for my child (enter child's (children's) name).

Allergies: (enter any allergies that your child or children may have)
Contact information: (enter cell phone number or numbers where you can reached while away)
Other Emergency Numbers: (enter name and phone number of another person who can notify you in case you cannot be reached)

Address: (enter you mailing address)
Date: (enter the date)
Signature: (print out and sign on the line)
There you go. You are all set and can rest assured that your child will get the necessary medical attention if need be in your absence.
